How do you sew a Hong Kong seam finish neatly? My bias tape always twists.

I want to use Hong Kong finish as my standard seam finish for unlined garments but the bias tape always twists at some point along the seam edge and I cant get the fold to stay even. Im cutting my own bias tape. Is this a cutting issue or an application issue?
